The importance of the patient story cannot be underestimated—it can be the difference between life or death. As patients, we need to ensure that our own story doesn’t get lost or overlooked when it comes to the health care we receive; as caretakers, we do that for our loved ones; and as healthcare documentation specialists, we advocate for all those whose medical records we touch.
Acknowledging that documentation errors have the potential to cause patient harm and that those errors happen far too frequently is the first step in reducing avoidable adverse events.
